YOUR ROLE

Our Care Partner team identifies and addresses barriers to care and the social determinants of health using phone, text, video, and email. As a Sr. Manager, Care Team your job is to lead the day-to-day functions of our Care Partner and Administrative Assistant teams, including operational management, escalation support, performance management, and strategic growth. You will report to our Director, Care Delivery and work cross-functionally with our Product, Operations, Quality, and Training teams.

Throughout your workday, you will:

  • Provide operational oversight of Care Partner and Administrative Assistant work
  • Support escalated member issues for non-clinical needs
  • Lead performance development and educational sessions for our Care Team Leads
  • Monitor, execute, and enhance performance monitoring efforts
  • Assist the team in supporting members to overcome barriers to care. (e.g., health literacy, unstable housing)

After your first three months, you will:

  • Have completed virtual onboarding and training and be up to speed on Thyme Care systems, tools, technology, partners, and expectations. You’ll spend your day supporting our Care Partner and Administrative Assistant Team Leads.
  • Have built strong, trusting relationships with your team, where listening and empathy are the foundation for every interaction.
  • Be able to independently lead, coach, and grow your team.
  • Cultivate a culture of growth mindset, continuous learning and improvement, and service delivery excellence.
  • Execute Care Team policies and procedures, escalation pathways, best practices, and highest standards, including team efficiency metrics and quality outcomes oversight.
  • Lead professional development sessions, teaching others to apply feedback and best practices to meet productivity and quality goals.
  • In collaboration with your Leader and Clinical leadership, build a Community Health Worker-based training and development program for the Care Team.

WHAT LEADS TO SUCCESS

Your resume and prior roles reflect:

  • At least 5 years in a Care Delivery leadership role leading other leaders. Your experience should include overseeing member-facing or patient-facing navigation or care coordination teams with a demonstrated ability to drive outcomes in a metrics-driven, feedback-oriented environment.
  • Experience delivering services to patients with complex clinical and social needs
  • Community Health Worker, Social Work, or extensive experience working with underserved, high-need populations, preferred
  • Medicare, Medicare Advantage, and Medicaid experience, a plus.
  • A member-first approach.

  • Operator mindset. You can break things down into actionable steps and execute plans with ease. You’re comfortable surfacing blockers to plans and pivoting based on real-time feedback as needed.
  • A quiet working space. It’s important that you’ve worked in a remote role in the past. To ensure member privacy, you must operate in a quiet and secure environment and be able to test your internet speed before starting to ensure our applications function as expected.

Our salary ranges are based on paying competitively for our size and industry and are one part of the total compensation package that also includes equity, benefits, and other opportunities at Thyme Care. Individual pay decisions are based on a number of factors, including qualifications for the role, experience level, skillset, geography, and balancing internal equity relative to other Thyme Care employees. In accordance with New York City law, the base salary for this role if filled within New York City, is $125,000-$150,000. The salary range could be lower or higher than this if the role is hired in another location or at another level. We also believe that your personal needs and preferences should be considered, so we allow some choice between equity and cash.
