Imagine building a better healthcare journey for cancer patients, where individuals and their loved ones feel seen, supported, and heard by their care team – both in and out of the clinic. Where fast access to high-quality care is the norm, not the exception. Where patients have access to a care navigator to guide them through their diagnosis, and trusted support all along the way.

At Thyme Care, we share a passion for transforming the cancer care experience – not just for patients but also for their caregivers and loved ones, as well as those delivering and paying for their care. Today, Thyme Care is known predominantly as a cancer care navigation company enabling value-based cancer care; in the next few years, we will become a nationally recognized technology-driven and provider-centric care delivery model, reshaping the landscape of cancer care access, delivery, and experience. Our commitment runs deep—we're not satisfied with the status quo but determined to redefine it.

To make this happen, we’re building a diverse team of problem solvers and critical thinkers to drive innovation and shape the future of healthcare. Additionally, you'll facilitate weekly case conferences, offering guidance and collaboration opportunities for the nursing team to optimize patient care plans. Your schedule will typically encompass Monday to Friday shifts, with one weekly late shift allocated for urgent care coverage, collectively defined by the team. • Provide clinical input and serve as an escalation point for patient care within designated pods.
  • Review patient information to determine appropriate levels of care upon entry to Thyme Care.
  • Engage in chart reviews, care plan creation, and clinical guidance for oncology-trained nurses.
  • Participate in urgent care shifts as needed, typically limited to 1-2 weekly shifts, without weekend or after-hours responsibilities.
  • Collaborate with the daytime APP leader and three Medical Directors for support and escalation needs.
  • Prioritize patient needs, focusing on triage to prevent hospital admissions and addressing social determinants of health.

  • Member-Centric Approach: Demonstrating a commitment to member care and empathy in every interaction.
  • Action-Oriented: Proactively identify and address member needs, taking prompt and effective action.
  • Organizational Skills: Excelling in multitasking and maintaining meticulous organization in care coordination and documentation.
  • Communication Skills: Building strong rapport and effective working relationships with clinical and non-clinical colleagues.
  • Adaptability: Thriving in dynamic environments and embracing change with agility and effectiveness.
  • Qualifications: A Nurse Practitioner with relevant experience, a Bachelor of Science in Nursing, and a compact unrestricted Registered Nurse (RN) license in a compact state. A minimum of 3 years of experience in adult oncology is required. Additionally, possessing certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) is a big plus!
