Utilize Health is an early-stage care management company specializing in populations with complex needs. We help individuals who have a neurological condition such as traumatic

brain injury, spinal cord injury and stroke.

JOB OVERVIEW

The Utilize Health enrollment specialist is responsible for developing and nurturing relationships with community partners, enrolling new members, and facilitating a collaborative approach to outreach and enrollment. They will engage with our community partners to initiate contact with new members, making field visits and educating members on the advantages of our neuro care program. They are responsible for ensuring a seamless member enrollment experience. Must be located in Nashville, Tennessee area.

JOB RESPONSIBILITIES

Registration and Enrollment - Customer Service

  • Ability to explain the program offered in an informational, influential, concise, and personable manner.
  • Demonstrate professional etiquette when speaking with members by phone and in person.
  • Enrollment & registration of new members including demographics, insurance, and enrollment/initial visit.
  • Support and continuously improve operations to scale UH member outreach and enrollment.
  • Provide members and prospective members with education about UH programs.
  • Promptly address member concerns and work with team to obtain resolution.
  • Promptly refer members to clinical team for clinical escalations.
  • Visit members in their home/community.
  • Mail member letters and perform other enrollment duties as needed.
  • Ensure a seamless member experience from enrollment through discharge.

Appointment Scheduling & Staffing Templates

  • Coordinate member outreach and manage appointment schedule – make, change, and cancel appointments; verify appointments.
  • Collaborate with clinical team to ensure efficient scheduling.
  • Assist with development and implementation of efficient scheduling and staffing matrixes by geographic visits.
  • Management medical records for new referrals.
  • Other duties as assigned

QUALIFYING JOB KNOWLEDGE, SKILLS AND ABILITIES

  • Bachelor’s degree required.
  • 2+ years working in care management, social work, or home health.
  • Excellent customer services skills
  • Must be proficient in Microsoft Office Suite and have the ability to learn EMR system; eCW experience a plus.
  • Excellent oral and written communication skills
