Care managers and nurses serve as advocates, helping members navigate their healthcare journey by addressing physical, behavioral, functional, preventative, and social determinants of health (SDOH) needs and coordinating with members and their care teams.

Role Description

This is a full-time, hybrid Care Manager role based in Clarksville, TN. The Care Manager will be responsible for providing advocacy and personalized care management to individuals with complex neurological conditions. Responsibilities include conducting case management, supporting members with their healthcare needs, connecting members to appropriate resources, coordinating with healthcare providers, and facilitating continuity of care. The Care Manager will also play a key role in addressing the physical, behavioral health, and social needs of members to improve their overall quality of life.

Qualifications

  • Experience in Case Management and delivering compassionate care to individuals
  • Proficiency in training and educating clients, caregivers, or staff on care-related needs
  • Knowledge and experience with Elder Care and Social Services to address diverse member concerns
  • Familiarity with medical care practices, especially related to neurological or chronic illnesses
  • Strong interpersonal, organizational, and problem-solving skills
  • Bachelor’s degree in Social Work, Nursing, or a related field is preferred
  • Professional licensure or certification (e.g., RN, LCSW, or equivalent) is an asset
  • Prior experience working in care management or for a healthcare organization is highly beneficial
