Clinical Quality Nurse ManagerHours: SalariedReports to: Clinical OperationsABOUT USUtilize Health’s mission is to improve the health and well-being of people with neurological conditions through specialized care management for brain injury, stroke, and spinal cord injury. Care Managers and Nurses support members across physical, behavioral, functional, preventive and social determinants of health needs.JOB OVERVIEWEnsures quality throughout the Utilize Health patient journey. Collaborates with care managers to ensure optimal patient care coordination. Monitors and intervenes on patient care measures (gaps in care, hospitalizations, clinical indicators). Responds to clinical escalations, creates/reviews care plans, overcomes barriers to care, supports patient/family education, and addresses social determinants of health. Acts as a patient advocate to improve satisfaction and outcomes.JOB RESPONSIBILITIES- Member of multidisciplinary team; provide care coordination, health promotion, care planning, and transitional care.- Reduce hospital/ER admissions; close gaps in care; coordinate care for stroke, SCI, and brain injury populations.- Track quality measures and intervene to improve.- Continually assess needs; develop individualized care plans with measurable goals; ensure daily medical, behavioral, and environmental needs are coordinated.- Demonstrate knowledge of community/care resources; participate in patient identification/enrollment, huddles, case reviews, triage, and warm handoffs.- Liaison between patients and providers (PCPs, specialists); develop and disseminate patient education and action plans; support transitions of care.- Ensure timely documentation in the EHR.OTHER RESPONSIBILITIES- Maintain an active Tennessee Registered Nurse License.- Attend in-services, continuing education, and meet annual health requirements.- Comply with OSHA/safety and HIPAA requirements.- Perform other duties as assigned.BENEFITS- Medical, Dental, Vision; HSA; Basic Life and AD&D; Short- & Long-Term Disability; Supplemental Life, Cancer, Accident, Critical Illness; 401k; PTO; Calm app; Gym discounts.QUALIFYING JOB KNOWLEDGE, SKILLS AND ABILITIES- Graduate of an accredited RN program; Compact or Tennessee active RN licensure; AHA BLS required upon hire.- Two years clinical quality experience preferred; case management or home health preferred.- Ability to travel; neurological care experience; passion for neuro population; strong care coordination and EMR documentation; excellent communication; Microsoft Office proficiency.APPLYSend your resume and cover letter with the subject “Clinical Quality Nurse Manager Application” using the link below.
